LCWTL Forms
Many of the forms that used to be on this page were Procedural in nature and have been incorporated into the new LCWTL Handbook.
Some forms are in Microsoft (.doc) or (.xls ) format so you may fill them out online and print. Other forms that cannot be edited are in Adobe (.pdf) format. You must print these and then fill them out.
Download Microsoft Word Viewer to view WORD (.doc) files if you do not have Microsoft WORD.
Download the correct version of Adobe Reader for your operating system to view any .pdf files.

| 29 | Proposed Rule Change Submission (.docx) | Any League member can use this form to submit a rule change by a date determined by the Executive Officers. Downloads to your computer. |
