LCWTL Forms

Many LCWTL forms are required at different times of the year. Other forms are to help you manage your team.

Some forms are in Microsoft (.doc) or (.xls ) format so you may fill them out online and print.  Other forms that cannot be edited are in Adobe (.pdf) format. You must print these and then fill them out.

Forms for Captains
Form Name Use this form to:
1a Team Registration Instructions (.pdf) register your team online.
1b Player Registration Instructions (.pdf) register your players online. Money sent as directed by the league.
1c Captain's Worksheet (.docx) help captain's gather information about their team.
4 Rain Delay Form (.pdf) record players, positions and scores in case there is a rain delay. Print a copy to carry with you at all times.
6 Score Sheet (.doc) fill out your team online and then print.
6a Score Sheet Sample (.pdf) This is an example of a team score sheet you would have at a match.
6b Preparation of Line Ups and Score Entry (.pdf) to find out how to enter scores.  Home Captain enters scores.  Visiting Captain verifies scores.  ***Important that individual players check the scores for their matches.
8 Change in Roster (.doc) add, delete, or transfer players after the initial player registration has finished

11 Expense Report (.doc) fill out and file an expense report and send to Treasurer.

16 Default Record (.doc) record your Division defaults and submit to the LCWTL at the end of the season. For Division VPs.

3 Appeals Criteria (.pdf) read the details surrounding whether you should apply for an appeal.
3a About Appeals (.pdf) learn about the Appeals process.
7 Request for Medical Appeal  (.doc) fill out to file a Medical Appeal. Send to the director Sept 1-30 before seasonal play begins. You can edit this file online.
7a Request for Medical Appeal (.pdf) print this file and fill out by hand to file a Medical Appeal. Same as Form 7.
12 Medical Emergency Information (.doc) fill out emergency contact and medical information and carry with you at all times.
21 Match Win/Loss Record to record your own personal wins/losses for the season.
